Obtaining Medals and Military Records


Medals:
Medals and ribbons that were awarded while in active service will be issued to the former service person or his/her representative upon request. When requesting awards and decorations, please include your military service number, your Social Security Number, a copy of your separation papers (DD-214 or WDAGO 53-55), your complete mailing address and daytime phone number.

Military Records:
A copy of your military records can be obtained by contacting our office for the SF-180 form. Or you can write the National Personnel Records Center, 9700 Page Blvd., St. Louis, NO 63132.

List the records that you need, your branch, service number, dates of service, a mailing address, home phone number, and any information that will help them find your records.

If you received a bonus for military service in time of war, we can usually recover your separation papers in a few days from the state that you entered the service from.

Our office has the emergency phone numbers for the various branches, if a veteran's records are needed for medical care.


Review of Discharges:
Each branch of service maintains a Discharge Review Board with authority to change, correct, or modify discharges or dismissals that are not issued by sentence of a general court-martial.

Our office will be happy to assist veterans in their review requests. Remember that no discharge is automatically upgraded - You must apply.
